Please remember that your registration forms can be mailed-in or dropped-off in our convenient Faith Formation drop-off box located outside of the Rectory.

Sacrament Details:
- First Reconciliation / First Eucharist is offered to children in Grade 2 or higher. Children are required to have completed the preceding year's curriculum.
- Confirmation is offered to children in Grades 8 through Grade 12. Children are required to have completed the preceding year's curriculum.

Registration Fees:
- The registration fee for each child is $200.
- There is an additional fee of $100 for children preparing to celebrate First Eucharist and Confirmation.
- There is a late registration fee of $100 per child for anyone registering after August 31st.
- A full refund will be made if registration is cancelled prior to classes starting.
Payment Details:
- Please make check payable to "St. Catherine of Siena."
- Mail to: St Catherine of Siena, Attn: Faith Formation Office, 4 Riverside Avenue, Riverside, CT 06878
- Drop off: St. Catherine of Siena Rectory.
- Monday - Friday from 10am-3pm.
- If after hours, please put it in an envelope (Attn: Faith Formation) and leave it in the blue milk crate near the front door of the rectory.
Please complete all applicable forms, include payment and mail or drop off entire packet, per the options outlined above.
